Employment Benefits

5 Star Cares offers a comprehensive benefits package for full-time employees:

Health Insurance
We offer three healthcare plans for you to choose from: Optima Vantage (POS 1000 / 25 / 30%), Optima Vantage (POS 500 / 20 / 20%), and Optima Vantage (POS Equity 3000 100% HSA). Employees contribute to the premium cost, and the company covers 100% of employee-only coverage for eligible employees of the HSA plan. Dependent coverage is available with additional contributions.
Dental Insurance
Our dental plan, provided by Lincoln Financial, covers 100% of preventative services, 80% of restorative services, and 50% of major services. Employees pay 50% of the premium for Employee-only coverage. The annual benefit limit is $1,250.00, with a small deductible of $50.00 per individual ($150 max per family).
Short-Term Disability Insurance
We provide Short-Term Disability insurance at no cost to you. This coverage, through Lincoln Financial, pays 60% of your wages up to $750 per week for a maximum of 13 weeks after an 8-day waiting period. If your disability is caused by an accident, benefits begin immediately.
Long-Term Disability Insurance
Long-Term Disability insurance is provided at no cost to employees through Lincoln Financial, covering 60% of your monthly income up to $6,000. Benefits begin after 90 days following Short-Term Disability.
Benefit Eligibility
Employees are eligible for benefits starting the first of the month after 90 days of employment.
